Your passport must be a valid one.
You must submit a covering letter for the intent of permanent residency.

Form 21, duly completed and signed by the applicant.
Fee of P500.00 (around $4320.30 USD) and receipt.
2 recent coloured passport sized photographs of the applicant.
If married, include the following: certified copy of marriage certificate and certified copy of the birth certificate for children (as applicable).
Certified copy of Residence permit/Exemption certificate valid for at least not less than 3 months.
Reference letter/confirmation of employment by the employer.
Covering letter/application letter from the applicant.
Certified copy of Passport.
