Receipts which prove you have paid all applicable fees.

Letter from your employer describing the purpose of your stay in the U.S which must fall into the I visa activities. In addition, the letter should detail your length of stay, and the duration of your work contract, as well as your personal information.

Your qualifications which ensure you fall into the I visa category: Work contracts, Journalistic accreditation, Press card, and Past published articles.

Medical documents proving you are in good health.

Documents proving you have no past criminal record.

You need a passport valid for at least 6 months.

You must represent a foreign information media outlet.

You must have authorization through the Electronic System for Travel Authorization (ESTA) prior to boarding a US-bound air or sea carrier.

A valid Electronic Machine Readable Passport with at least 6 months expiry.

Proof that you have extraordinary abilities (this should be the same proof that your employer has handed into USCIS).

If you are planning to bring your family, you must submit proof of your relationship to them (marriage certificate for your spouse and birth certificates for your children).

You must demonstrate extraordinary ability by sustained national or international acclaim.

You must be coming temporarily to the United States to continue work in the area of extraordinary ability.

Extraordinary ability in the fields of science, education, business, or athletics means a level of expertise indicating that the person is one of the small percentages who has risen to the very top of the field of endeavor.

Extraordinary ability in the field of arts means distinction.  Distinction means a high level of achievement in the field of the arts evidenced by a degree of skill and recognition substantially above that ordinarily encountered to the extent that a person described as prominent is renowned, leading, or well-known in the field of arts.

Or, you must demonstrate extraordinary achievement evidenced by a degree of skill and recognition significantly above that ordinarily encountered.

Form I-485, Application to Register Permanent Residence or Adjust Status

Copy of the Form I-797, Approval or Receipt Notice, for the Form I-140 filed on your behalf (unless you are filing your Form I-485 together with the Form I-140)

Form I-485 Supplement J, to confirm that the job offered to you in Form I-140 remains a bona fide job offer that you intend to accept once your Form I-485 is approved

Copy of your government-issued identity document with photograph

Copy of your passport page with nonimmigrant visa (if applicable)

Copy of your passport page with admission or parole stamp (issued by a U.S. immigration officer) (if applicable)

Copy of Form I-94, Arrival/Departure Record, or copy of the U.S. Customs and Border Protection (CBP) admission or parole stamp on the travel document (if applicable) 

Proof that you have continuously maintained a lawful status since arriving in the U.S. (or that you are exempt under INA 245(k))

Form I-864, Affidavit of Support Under Section 213A of the INA (only if your Form I-140 petition was filed by a relative who is a U.S. citizen or lawful permanent resident

Form I-693, Report of Medical Examination and Vaccination Record (you may submit this form together with Form I-485 or later, such as by mail when we request it or in person at your interview, if any)

Certified police and court records of all criminal charges, arrests, or convictions regardless of final disposition (if applicable)

Form I-601, Application for Waiver of Grounds of Inadmissibility (if applicable)

Form I-212, Application for Permission to Reapply for Admission into the United States After Deportation or Removal (if applicable)

Documentation of past or present J-1 or J-2 nonimmigrant status (if applicable), including proof of compliance with or a waiver of the 2-year foreign residence requirement under INA 212(e)

Form I-485 Supplement A, Adjustment of Status Under Section 245(i) (if applicable)

You must be capable, at the time the petition is filed on your behalf, of performing unskilled labor (requiring less than 2 years training or experience)

Your employer needs to demonstrate an ability to pay the offered wage

You need a Labor certification and a permanent, full-time job offer from an employer in the United States

Copy of the principal applicant’s selection letter for the diversity visa lottery from DOS (You can get it on the DV lottery website when you get selected).

Two photographs per person of you and your family if you are applying together.

Birth certificates of you and your family (including copies).

Marriage certificates or proof of a divorce (if applicable).

Military records if you were in the military.

Copy of the receipt from DOS for the diversity visa lottery processing fee.

Copy of passport page with nonimmigrant visa (if applicable).

Any additional documents that the US Embassy might require from you.

You must be from one of the countries allowed to apply for the DV Program.

You must be at least 18 years old to apply as the head of household. This means that if you are over 18 years old but have younger children, then you can apply together.

You must have completed high school and have a high school diploma.

Or, you must have at least 2 years of work experience in the last 5 years in an occupation which falls into Job Zone 4 or 5 as per the US Department of Labor (DOL) requirements.

You must go through the medical examinations prior to the visa application.

To prove your relationship you can show documents such as You can use things like:  a marriage certificate or civil partnership certificate a tenancy agreement, utility bills or council tax bills confirming that you live at the same address or pay bills together a bank statement from a joint bank account, or confirming that you live at the same address a letter from your doctor or dentist confirming that you live at the same address.

Valid passport, pictures, payment of government fee.	

You need to prove that you’re in a civil partnership or marriage that’s recognised in the UK; you’ve been living together in a relationship for at least 2 years when you apply; you are a fiancé, fiancée or proposed civil partner and will marry or enter into a civil partnership in the UK within 6 months of arriving.

You need to have a good knowledge of English.

Can financially support yourself and your dependants.

Form I-485, Application to Register Permanent Residence or Adjust Status if you are currently in United states

Proof that you have continuously maintained a lawful status since arriving in the U.S. (or that you are exempt under INA 245(k)

Form I-864, Affidavit of Support Under Section 213A of the INA (only if your Form I-140 petition was filed by a relative who is a U.S. citizen or lawful permanent resident, or by a for-profit entity if 5% or more of the ownership interest is held by a relative who is a U.S. citizen or a lawful permanent resident)

Form I-212, Application for Permission to Reapply for Admission into the United States After Deportation or Removal (if applicable

Form I-485 Supplement J, to confirm that the job offered to you in Form I-140 remains a bona fide job offer that you intend to accept once your Form I-485 is approved.

You must demonstrate international recognition for your outstanding achievements in a particular academic field

You must have at least 3 years experience in teaching or research in that academic area

You must be entering or have entered the United States in order to pursue tenure or tenure track teaching or comparable research position at a university or other institution of higher education

You need a job offer from an employer in US
